The Full Outline of Unresponsiveness (FOUR) Score was developed to overcome the drawbacks of the widely used Glasgow Coma Scale. This scale includes more clinical information on the neurological status of the patient, including assessment of the respiratory pattern and the brainstem reflexes, but not the verbal response. It can therefore be used consistently, even in intubated patients. As a result, it is now being increasingly accepted and utilized, mainly, but not only, in intensive care units (ICUs). Until now it was not available in Greek. In this article, the official translation of the FOUR Score into Greek is presented. In order to facilitate its use, its special features are analyzed and application guidelines in Greek are provided. The FOUR Score is a useful and reliable clinical tool for the assessment of the level of consciousness, which is now available for Greek-speaking health care professionals.

Key words: FOUR Score, Level of consciousness, Neurological assessment, Translation.
