Posterior capsular contracture is a common cause of pain on the posterior aspect of the shoulder, associated with restriction of internal rotation. This condition often is misdiagnosed. It is sometimes associated with subacromial impingement, pathology of the biceps tendon or tears in the glenoid labrum. When the posterior capsular contracture is treated successfully, the impingement pathology often resolves. Initial management of posterior capsular contracture should be conservative. Stretching programmes usually restore the normal range of motion and eliminate the pain. In resistent cases, arthroscopic posterior capsule release is indicated.
