In Greece, the national electronic health record (EHR) has not been adequately integrated as an organic functional element of the health care system. On the contrary, the national e-prescription system (e-PS) has been universally implemented, being the exclusive digital health service that defines the relationship between patients and physicians. This is a peculiarity for the country, not in agreement with international standards, which must be resolved with the implementation of a digital transformation. The EHR must become the main system defining the patientsphysicians relationship, with the e-PS functionally intergraded. As part of a broad spectrum of strategic digital health transformation, both, EHR and e-PS infrastructure, need to be upgraded and properly aligned. An important part of this upgrade would be a new tool comprising the necessary clinical codes and terms for managing physician's concise clinical diagnosis entries for every patient in the EHR and e-PS.

Key words: Electronic health record, Electronic prescription system, ICD-10 and ICPC-2 disease coding systems.
